I hereby confirm that in calculating my Total Monthly Expenses and Incomes, as set out above, I have taken into account (excluding those expenses already reflected on my payslip) all my monthly expenses, including but not limited to all taxes, insurances, pension payments, medical aid payments, obligations in terms of court orders, groceries, water, electricity and gas expenses, school fees, transport costs and any other living expenses.

Smart Construction utilises certain evaluative mechanisms in order to assess the Total Monthly Expense amount that you have provided above. In circumstances where Smart Construction estimates this amount to be more than the amount that you have provided, Smart Construction will proceed to utilise the higher amount in order to assess your application for credit.

I declare that the above information is both true and correct and this information together with other information will be used to determine whether I can afford the credit I have applied for.

INSTRUCTION AND AUTHORISATION BY THE CONSUMER
I agree that by signing this agreement and/or furnishing Smart Construction with my [left and/or right] fingerprint, I direct that, and give Smart Construction permission to:
* Contact anyone to confirm that the information that I have provided is correct;
* Be provided with details from any party about my financial status, banking details, including my credit records and payment history, and details on the National Loans Register and Credit Bureau;
* Give and receive information about my credit agreement and/or any future credit agreement entered into by me, to any party, including any credit Bureau, South African Fraud Prevention Services (SAFPS) and/or any other similar organisation, and to disclose / receive my personal information:
1- Where it is legally compelled to do so;
2- Where disclosure is made with my written consent or instruction.
3- Where the exercise of Smart Construction's rights and/or interest require disclosure
4- Where there is a duty to the public to disclose
* File any false, misleading or dishonest information supplied by me in my application to the SAFPS (and/or any crime prevention)
database; and for the sharing of this information amongst members of SAFPS;
* File any information relating to a facility which has/is being used for the purposes of avoiding liability through deception to the SAFPS
(and/or any crime prevention) database; and for the sharing of this information amongst members of SAFPS;
* Sell, cede, assign or otherwise transfer all or any of its rights and/or obligations under this agreement;
* Request me to disclose any of my personal information when I contact Smart Construction telephonically.
* Submit my fingerprint for verification to the Department of Home Affairs' National Identification System, or to any other relevant party,
so as to verify and confirm my identity.

I confirm that any information, whether in writing or otherwise, that I have furnished to Smart Construction during the conclusion of this agreement or pursuant thereto is both true and correct.

Authorisation for financial, employment and credit information
I understand that all my financial, employment, and credit information is confidential and I hereby consent to Smart Construction having access to my financial, employment, and credit information as is set out below.

Financial information

I instruct my bank to give Smart Construction any financial information relating to my bank account, such as confirmation of the account number and that I am the account holder, my bank branch and the type of account that I hold, and any other transactional information that Smart Construction may require in respect of my banking account.

Employment information

I instruct my employer to give Smart Construction details of my employment, such as confirmation and status of my employment, the amount I am paid, the day on which I am paid and the account into which my pay is deposited.

Credit information

I instruct any credit bureau or other similar organisation that has any information pertaining to me or my credit worthiness to release such information to Smart Construction. I also consent to Smart Construction using any information pertaining to me or my credit worthiness for the purposes of offering credit products to me. Notwithstanding the fact that this credit agreement may have terminated, I confirm that the authorisation and instructions that I have given in terms of this clause shall remain in force and effect until such time that I withdraw such authority and instruction by giving Smart Construction written notice of my withdrawal.
